Original prescription for Mrs. Howe of Auburn, Nebraska, handwritten in pencil by V.M. Boal, M.D., on March 4, 1915 (3/4/15).

The ingredients for the prescription included opium, belladonna, and coca butter, presumablyfor rectal suppositories,"one at night."
