    Description

    Vintage 1915 U.S. Internal Revenue Order Forms for Opium, Etc booklet with seven unused opium order forms and both covers.  The booklet originally contained ten order forms.  The forms are stamped with the name of the purchaser and they have 949 written in pencil by the registry number.  The booklet measures 9"x13 3/8" and it is in good used condition with some wear.  Check photo.
